Functional Capacity Assessments
Our Functional Capacity Assessments provide a detailed evaluation of a participant’s abilities, areas of difficulty, and overall independence in everyday activities. These assessments are thorough and grounded in evidence-based practice, incorporating recognised tools such as WHODAS 2.0, Instrumental Activities of Daily Living (IADL), and the Child and Adolescent Needs and Strengths (CANS) where appropriate.
Each assessment report is carefully prepared to inform NDIS funding applications, with clear recommendations that reflect the participant’s individual goals, circumstances, and required supports.
Functional Capacity Assessments
For Adults and Adolescents
$ 2,909.85
- Initial consultation and history review
- Thorough telehealth-based assessments
- Application of standardised clinical tools (such as WHODAS 2.0, IADL, CANS)
- Comprehensive, individualised report preparation
- Report review and completion promptly, typically within 10 days of your first session with an Occupational Therapist, where needed
Total time commitment:
approximately 15 hours, consistent with standard NDIS pricing and reporting guidelines
NDIS Access Requests (Limited Use)
NDIS Access Requests are intended for individuals who are not yet participants in the NDIS and need initial evidence of eligibility.
These requests tend to have a lower approval rate when the submitted evidence does not meet the necessary criteria.
As a result, Access assessments are provided privately on an upfront basis and may not be suitable for everyone.

NDIS Access Request Form
For Adults and Adolescents

Supported Independent Living/Specialist Disability Accommodation Assessments
Specialist Disability Accommodation (SDA) funding through the NDIS.
These assessments are most effective when evidence clearly demonstrates the participant’s functional needs and housing requirements. Insufficient or incomplete evidence may affect funding approval.
As a result, SIL/SDA assessments are provided on a private, upfront basis and may not be suitable for all participants.

SIL/SDA Assessment Package

- Functional Capacity Assessment (15 hours) including initial consultation, history review, and telehealth-based evaluation
- Telehealth assessment of the participant’s living environment (10 hours)
- Use of standardised clinical tools (WHODAS 2.0, IADL, CANS)
- Collaboration with specialists and other healthcare providers
- Detailed, individualised report preparation for NDIS funding purposes
